Clinical Readiness, Family Preparedness, and Transfer of Care: ILPQC’s Strategies to Optimize Coordinated Discharge for Opioid-exposed Newborns

This presentation will offer an overview of how the Illinois Perinatal Quality Collaborative (ILPQC) implemented their Mothers and Newborns affected by Opioids – Neonatal Initiative. It will focus on the initiative’s “Coordinated Discharge Planning” aim implementation strategies, which center around a comprehensive approach that includes clinical readiness, family preparedness, and transfer of care. The presentation will include information about the strategies and resources developed for the collaborative and how they build in PDSA (Plan, Do, Study, Act) cycles to improve messaging.
Panel Discussion: Denver Health/CHoSEN Perinatal Navigator Program
This panel will address details about provider roles, referral and work-flower processes, and the program’s successes, barriers, and challenges thus far from the perspectives of those working directly with clients and families.
