Overview
CPCQC Forum- Better Together will bring together hospital teams and engaged partners to learn how to improve perinatal outcomes. Our NEST, SOAR, SPARK, and Turning the Tide programs will provide focused presentations on topics of interest for each program. In addition, this will provide an opportunity to connect with other hospital teams and learn about the QI work happening around the state.
ACCME Objectives
- Analyze the complexities of the post-partum experience during the fourth trimester.
- Discuss how measurable outcomes and authentic relationships can coexist to strengthen perinatal care improvement efforts.
- Describe how coordination among prenatal, L&D, postpartum, and community-based providers influences birth outcomes and continuity of care.
- Identify barriers to reducing unnecessary cesarean births and primary cesarean births while maintaining safe, evidence-based intervention thresholds.
- Develop and implement individualized, patient-centered plans of care that address medical, mental health, IPV, and social needs while ensuring safe discharge.
- Describe and assess the impact of perinatal cannabis, nicotine, and alcohol use on maternal and infant health, including national and Colorado-specific trends.
ANCC Learning Outcomes
- Learners will self report greater awareness around practical strategies and actionable tools for reimagining communication and trust as essential drivers of sustainable, high-value care for mothers, babies, and families.
- Learners will self report increased knowledge and how coordination among prenatal, L&D, postpartum, and community-based providers influences birth outcomes and continuity of care across the perinatal period.
