Integrated Care Program Manager
Responsibilities:
Integrated Behavioral Health Program Management
- Lead integrated behavioral health program outreach, planning, implementation, and reporting
- Plan and facilitate external meetings with key partners
- Track grantee invoices, budgets, contracts, and work plans
- Maintain detailed documentation for integrated behavioral health programs and ensure timely communication with partners across local perinatal continuums of care
- Evaluate program outcomes and progress toward goals in partnership with the Director of Integrated Behavioral Health and CPCQC Data and Analytics team
- Prepare and submit grant reports
- Maintain and update program materials, including but not limited to program model, evaluation plan, one-pagers, and related resources
- Manage initiative calendars, including grant deliverables, deadlines, and educational and conference opportunities
- Evaluate and assess how health equity is prioritized across integrated behavioral health programs and work with Director of Integrated Behavioral Health to strengthen health equity conversations, data analysis, and outcome objectives
Community Engagement
- Co-design and implement community outreach strategies to strengthen connections with a diversity of partners and community members across Colorado regarding IMPACT BH and FIRST programs
- Support programs that engage community members and amplify their voices in maternal and infant health initiatives
- Engage with internal CPCQC teams’ connections to clinical teams to ensure community perspectives inform CPCQC initiatives
- Foster collaborations with statewide and community organizations to advance shared goals
Development and Communications
- Communicate CPCQC objectives to clinical and community partners, and share initiative-related information
- Enhance relationships with prospective and existing funders in partnership with Director of Integrated Behavioral Health
- Work with CPCQC’s communications and development team on:
- Social media content and thought leadership to raise awareness of CPCQC and partner work.
- Presenting at conferences across Colorado, and potentially nationally, on the impact and lessons of CPCQC’s initiatives.
- IMPACT BH grant program communications
Qualifications:
- 2–5 years of progressive leadership experience in the nonprofit or healthcare sector, with experience in maternal and child health initiatives preferred
- Bachelor’s degree (required) or master’s degree in mental health, public health, social sector initiatives, or related field
- Experience working with community-based organizations and healthcare providers on community-led initiatives
- Public health research, evaluation, analysis, and/or reporting experience
- Program and/or grants management experience, including budget tracking and reporting
- Proven operational leadership skills, including project management, strategic planning
- Excellent organizational, prioritization, and communication skills essential for coordinating multiple projects and partners
- Experience or comfortable using Google Suite, Adobe, and Asana
- Relationship-building skills to engage effectively with team members, partners, and community members
- Strong commitment to working alongside community members and partners to address perinatal health inequities
- Ability to work collaboratively in a virtual team-oriented environment.
- Desire to travel statewide to meet and build relationships with local communities regarding perinatal behavioral health needs and solutions
Salary and Benefits:
The salary range for this position is $79,000 – $83,000 annually, based on experience and qualifications. Benefits include unlimited paid time off, paid holidays, employer medical, dental, and vision coverage contributions, and a company 401(k) match.
